Emergency Address Form

As required by our upstream voice service providers, this form collects the physical address necessary to enable emergency call routing through their infrastructure to Public Safety Answering Points (PSAPs). The information you provide will be registered with the carrier to support emergency services (e.g., fire, police, EMS) for your location. By submitting this form, you acknowledge and agree that this address will be used to configure Emergency Services (E911) functionality for Makios Cloud Phones at the specified physical location. It represents the dispatchable location that will be transmitted to emergency responders in accordance with federal requirements, including Kari’s Law and the RAY BAUM’S Act.

Makios is a provider of managed business phone systems delivered via the cloud, not a telecommunications carrier. We do not route, manage, or terminate emergency calls, and we do not maintain direct relationships with Public Safety Answering Points (PSAPs). All emergency calling functionality is provisioned through our licensed upstream voice service providers. Makios enables the technical features within the Cloud Phones system to support compliance but assumes no liability for delays, routing errors, or failures resulting from incorrect, missing, or outdated address data provided by the client.

The client is solely responsible for providing accurate and complete location information for each physical site. The client agrees to notify Makios in writing within five (5) business days of any change, including but not limited to address updates, office relocations, or new phone number assignments. Emergency calling features will not be activated unless this information has been submitted and confirmed. Failure to comply with these requirements may result in delayed service activation, call failures, or emergency misrouting. The client assumes full responsibility for any such outcomes.
